Franklin Foundation Hospital Benefits’ Preview
Franklin Foundation Hospital offers a wide variety of employee benefits to full-time and regular part-time employees. Each employee is able to choose those benefits that satisfy their needs, and at a reasonable price. Each benefit has its own eligibility requirements and terms and conditions. Most benefits have a ninety day waiting period; however, there are some that are effective immediately.

Franklin Foundation has a 125k Cafeteria Plan. This is a plan that allows certain benefits to be pre-taxed. In other words, employees do not pay taxes on their premiums for covered benefits. You only pay taxes when you receive the benefits.

Employee Group Medical – your medical coverage is provided by the Aetna Life Insurance Company. Your plan has four tiers: employee, employee and spouse, employee with children(s), and family. Annual premiums are shared by you and Franklin Foundation Hospital. You must be employed by the hospital for ninety days to become eligible to participate.

Employee Dental - is a voluntary benefit. Your provider is Ameritas. You may purchase dental insurance for yourself and family.

Employee Group Life Insurance - group life insurance for employees is purchased by the hospital. Employees are covered at one and a half times their straight time annual income.

Employee Retirement Plan – employees may contribute to their retirement plan beginning the first pay period after they have been hired. You can contribute from 0% up to 100 % of your salary or wages. Your contributions are pre-taxed, in other words, you do not pay taxes on your contributions until you take distribution, except for rollovers. Franklin Foundation Hospital will also match the first 100% of the first 2% of your
compensation. By law, certain limits apply to your compensation and contributions. Your contributions will always be yours, however, the matching contributions made by the Franklin Foundation Hospital will be vested beginning after the first year of service and fully vested once you have achieved five years of service with the hospital.

Employee Long Term Disability – is a voluntary benefit. Your provider is the Aetna Life Insurance Company. Employees pay the entire premium.

Employee Voluntary Life and Accidental Death and Dismemberment - is a voluntary benefit. Employees can purchase additional life insurance on himself and coverage for his spouse and/or children.

Aflac Plans - Aflac has a variety of insurance protection plans which you may choose from. All premiums are at group rates which make them affordable. Covered plans are:

Cancer Indemnity
Short Term Disability
Accident Indemnity
Special Event Protection
Hospital Indemnity
Vision

Each plan has its on set of terms and conditions.

On the date of your initial orientation a Human Resource representative will give you an overview of each benefit, and during your benefits orientation, our benefits’ coordinator will discuss in detail each benefit and its cost.
